Severity — worst‑case ranking across User Safety, Environmental, GMP/Product, Production, Cost and Energy. A severity of 0 is permitted where an out‑of‑tolerance condition has no impact.
Detectability — ranked by whichever detection mode applies to the instrument: automated online inspection, or manual/human verification.

The ISPE guide's own worked example (Appendix 1, Tables 10.1–10.4) uses a simpler 3-tier High/Medium/Low system. It is reproduced below for cross-checking — the two systems are not on a 1:1 numeric scale, so treat this as a sanity check on the FMEA result above, not an alternate calculation.
